Middle cerebral artery peak systolic velocity for the diagnosis of fetal anemia: the untold story.

چکیده
The use of the middle cerebral artery peak systolic velocity (MCA-PSV) for the diagnosis of fetal anemia has been one of the few discoveries in fetal medicine that is changing the standard of care in the management of affected pregnancies1. This has led to a more than 70% reduction in the number of invasive tests, which often cause fetal death, in the assessment of red-cell alloimmunized pregnancies1. Up to 2 years ago, I thought that only a few doctors in the world were relying upon the MCA-PSV for the diagnosis of fetal anemia. However, in 2003 at the Society for Maternal–Fetal Medicine (SMFM) meeting in San Francisco and a few months later at the American Institute of Ultrasound in Medicine (AIUM) meeting in Montreal, during my lectures on fetal anemia, I asked my colleagues, ‘how many of you are using the middle cerebral artery peak systolic velocity in fetuses at risk of anemia?’, and I was pleasantly surprised to see that about 70% of the participants raised their hands. I then realized that the use of the MCA-PSV in fetal medicine had become a reality.

Accurate assessment of gestational age, fetal growth, and the detection of fetal and placental abnormalities are major benefits of sonography. Color Doppler can be used to assist in the identification of vascular architecture, detection of vascular pathology and visualization of blood flow changes associated with physiologic processes and disease states. The clinical applications of obstetrical...
متن کاملA Study of Fetal Middle Cerebral Artery Peak Systolic Velocity to Diagnose Fetal Anemia in Rural Obstetric Population
Globally, fetal middle cerebral artery peak systolic velocity (MCA-PSV) is used for noninvasive diagnosis of fetal anemia. Although, incidence of anemia is common in rural setup, scientific studies describing utilization of this modality are lacking. This study throws light on how the use of fetal MCAPSV to diagnose fetal anemia can help in the decision-making process even in a rural setup. Con...

Objective: To evaluate the screening efficiency of ultrasound and Doppler measurements in the prediction of fetal anemia in alloimmunized pregnancies. Materials and Method: In a prospective study, 24 nonhydropic fetuses with red blood cell alloimmunization were evaluated with ultrasound and Doppler imaging. Middle cerebral artery peak systolic velocity, intrahepatic umbilical venous maximum vel...
متن کاملTranscranial doppler sonography diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine
Background: An imbalance of the cerebrovascular response during functional activation of the brain has been postulated as a factor in the pathophysiology of migraine. The purpose of this study was to determine the transcranial doppler sonography (TDS) diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine. Methods: This study was carried out on 46 pa...

BACKGROUND Fetomaternal hemorrhage is a rare cause of fetal anemia and hydrops fetalis. Early and severe fetomaternal hemorrhage may benefit from in utero transfusion(s); however, hemorrhage rate is unpredictable, and reliable criteria are needed to identify recurrent anemia. CASE Fetal hydrops due to massive fetomaternal hemorrhage was diagnosed at 29 weeks. After the first in utero transfus...
